Appointments

At Pathways, we wish to make your experience as comfortable as possible. Many families ask us what to expect and how to prepare for a consultation. The following are some requirements and tips for making your appointment go smoothly.

Before you can make an appointment, you must first complete, copy (for yourself), and return a Patient Intake Packet or an Adult Intake Packet. Dr. Hicks must have an opportunity to review the patient’s history before the appointment. This allows us to:

  • Be familiar with the patient's health history.
  • Compile a list of directed and specific questions in advance.
  • Use the one hour consultation for precise examination, questions and treatment options.
  • We strongly recommend that the patient submit a copy of their medical records along with the intake form. 

For children, an immunization record is extremely important. If faxing us a large medical history, please do so after hours so that our fax is not made unavailable for business. Fax: (262) 740-3001.

Once we receive the intake, we will contact you to schedule an appointment. In making your appointment, you will have the following options:

  • Office visit in Delavan, Wisconsin. Call (262) 740-3000.
  • Office visit in one of our other offices and particularly scheduled times.
  • Phone consult (please note the fee is the same as an office visit). If Dr. Hicks does not have an opportunity to meet the patient and there are specific symptoms that you wish for him to see, we recommend sending pictures with the medical records or brief video footage. (Billed as phone consult).
  • Web appointments in your home. This is ideal if you have a web camera, but it is not necessary. The added bonus is the ability to request the consult on a CD for future viewing. You will receive an email from our office with a link to the meeting for your appointment. Instructions will be given in the email. (Billed as office visit).
  • Web appointments in one of our outreach offices. Just arrive at your scheduled time and you will be brought to an office set for a web appointment with Dr. Hicks. Our staff member will supply you with material and go over any questions you may have after the appointment. (Billed as an office visit).

There are a few things to be aware of before booking your stay.

  • Rates are $99 a night for a One Bedroom, regardless of the time of year.
  • For a Two Bedroom, $131 a night, regardless of the time of year.
  • Rooms have a kitchen with sink, microwave, and refridgerator. Dishes are provided.
  • You must bring your own pots and pans, and request a griddle or burner at check-in.
  • Free High Speed Wireless internet available throughout the building.
  • Indoor and Outdoor pools.
